Impact of TRALI on the cost-effectiveness of solvent detergent-treated plasma.

A poster presented at the BSH meeting in Glasgow highlighted recent findings that the cost-effectiveness ratio of solvent detergent treated plasma is significantly better than previously reported when non-infectious complications, in particular Transfusion Related Acute Lung Injury (TRALI), are taken into consideration.

TRALI is a severe, potentially fatal transfusion reaction caused by donor leukocyte antibodies. It is characterised by chills, fever, breathing difficulties and low blood pressure.
According to the UK Serious Hazards of Transfusion (SHOT) report TRALI is the second most frequent cause of transfusion-related morbidity and death. The literature suggests the incidence of TRALI is around one in 2-5,000 of fresh frozen plasma units transfused, although under-reporting and misdiagnosis are widely acknowledged.

By contrast there have been no cases of TRALI reported following transfusion of solvent detergent treated plasma.

Georg Riedler, presenting the poster on behalf of the authors concluded that in view of these findings, "a more comprehensive analysis ... is warranted to more accurately assess the cost-effectiveness of SDFFP".
